Each of our offices has a schedule tailored for your convenience.If you believe you can benefit from one of our services, your first step is to discuss this with your physician and obtain a prescription. Some insurance companies may also require you to obtain a Referral from your primary care physician or, obtain authorization directly from your insurance provider prior to scheduling an appointment. Please check with your insurance provider to see if you need this type of referral. Next, if you wish to receive home health services, please call 314.352.7889 to make an appointment. If you wish to make an appointment at one of our out-patient facilities for physical and/or hand therapy, select the facility most convenient to you from our Facility Locations Page. Contact the office directly to set-up your appointment. When you call the office, please have your prescription and insurance card(s) handy because we will need information from these to schedule your initial appointment. Once you have made your initial appointment, you may complete our registration paperwork prior to your first appointment by downloading and printing the three forms below. This will help to speed up the patient registration process upon your arrival and get you in sooner. If the patient is under the age of 18, one of his/her parents or legal guardian will need to come to the first appointment to authorize care.
