    Scheduling Your Appointment...

    If you believe you can benefit from physical therapy, hand therapy, home health services, industrial services, or women’s health services, here is what you will need to schedule an appointment with one of our offices:
    • First, obtain a prescription / referral from your physician, osteopath, podiatrist or dentist. Any of these medical professionals may write you a prescription for physical/hand therapy or home health services, much like they may write a prescription for medication. This needs to be signed by one of these medical professionals, dated, and include your medical diagnosis.

      By state law, our staff can only perform rehabilitative care to you on the prescription/referral of one of these medical professionals.

      Some insurance companies may also require you to obtain a Primary Care Referral from your primary care physician or obtain authorization from the insurance company prior to scheduling an appointment with us.

      Please check with your insurance provider to see if you need this type of referral.


    • Next, select one of our locations most convenient to you.
      Go to our Facility Locations Page to locate a facility closest to you, get the phone number, hours of operation and print directions.


    • Then, contact the office of your choice to schedule the appointment.
      Call our office and identify yourself as a new patient. The receptionist will then ask you a series of questions that will allow our staff to be prepared for your first visit. Please have available at the time of your call:

      • Your prescription and/or referral from your medical professional

      • Your insurance card or billing information

      • Your social security number and date of birth



      The receptionist will schedule your first appointment at that time and provide you with any further information you may need. Other appointments may be made at the time of your first visit after seeing your clinician.


    What do I need to bring to my first appointment?

    Please have the following items at the time of your first visit:
    • Your prescription and/or referral from your medical professional, your insurance card or billing information, your social security number.

    • If the patient is under the age of 18, one of his/her parents will need to come to the first appointment to authorize care.

    • To save you time at your first appointment, please download and complete the two forms below (Patient Registration & HIPAA Compliance) Forms and bring them with you to your first appointment.
